About the job:
The Registered Nurse under the direction and supervision of the Clinic Supervisor, provides skilled nursing care to participants, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family teaching and communicates participant’s condition to primary caregivers, physician and Interdisciplinary Team.

Graduate of a school of professional nursing.
Current state Registered Nurses (R.N.) License
Cardio Pulmonary Resuscitation (CPR) certification required
A minimum of one year’s experience in working with the frail or elderly population required
Must have a valid driver’s license, proof of insurance and have means of transportation.
Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Preferred Qualifications:
Bachelors of Science Nursing (BSN) preferred.
Conducts an in person comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
Conducts an in person nursing re-assessment semi-annually and as needed.
Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
As needed provides orientation, initial and annual competencies, of, Licensed Practical Nurse (LPN), Certified Nursing Assistant’s (C.N.A.’s), and Clinic Clerk.
Performs competencies on appropriate day center staff as needed.
Performs general nursing care to participants. Performs or supervises the administration of prescribed medications and treatments in accordance with nursing standards.
Triages participants to determine priority of care.
