PRN Physical Therapist (PT)

Physical Therapist at Enhance Rehabilitation Enhance Rehabilitation is currently seeking qualified Physical Therapists to join our dynamic team. As a Physical Therapist, you will evaluate and treat patients in a skilled nursing facility (SNF) setting, developing individualized plans of care.

Provide excellent care for our patients

Deliver exceptional customer service to patients and family members

Collaborate with facility staff to ensure quality care

Evaluate new patients, develop care plans, and set measurable functional goals

Treat patients, address concerns, and educate them on injury, progress, and outcomes

Supervise Physical Therapist Assistants

Re‑evaluate patient progress, revise care plans, and reset goals

Complete timely and accurate documentation (evaluations, re‑evaluations, discharges, daily progress notes)
