Case Managers overall goal is to provide comprehensive recipient assessment, comprehensive care/service development, linking/coordination of services, monitoring/follow up of services, and reassessment of status of the individual service needs of the registered client, within the community and office setting.
Individual must have at least four (4) years of college credit in the field of psychology, social work or a related field. One year of experience working with mental health population is preferred.
Must have good oral and written communication skills and be
fluent in both English and Arabic .
Additional responsibilities:
Assist in determining client eligibility for program services.
Assess Consumer's needs for appropriate levels of care.
Complete Case Management assessment(s)
Develop Individual Plans of Service
Link consumer to all available resources for identified needs
Complete periodic reviews and discuss progress with the consumer, psychiatrist and specific clinical staff.
Maintain clinical files and records.
Attend all scheduled meetings and in‑service training seminars as required and assigned
Meet and Maintain direct service requirements.
Complete Peer Reviews and work in collaboration with the Quality Department to ensure the standards of the agency.
All other related duties as assigned.
