Job Overview The Patient Navigator will identify, coordinate and link Hepatitis B, C and HIV positive clients to medical care from all access points, primarily the Emergency Department. The navigator will identify positive patients, establish communication with them, and provide individualized support to overcome barriers to initial care. The role includes delivering health education to increase linkage success and serving as the primary contact and liaison for clients and the care team to ensure prompt and successful connection to first medical appointment.

Qualifications

• Bachelor's degree preferred or equivalent training in a related social or human service field

• Experience working in a healthcare setting with the community

Scheduling Requirements

• Day shift with occasional flexibility to accommodate community events and patient schedule preferences

Essential Functions

• Identify and engage appropriate patients

• Link and re-link new and previously diagnosed patients to medical care

• Provide patient education regarding diagnosis and treatment

• Facilitate care coordination, including arranging transportation, supporting insurance matters, accompanying patients to appointments, and providing reminder phone calls, texts, and emails

• Initiate appropriate supportive referrals such as substance abuse treatment, mental health, and PrEP

• Serve as primary liaison between screening, diagnostic, and treatment teams throughout the continuum of testing to initial treatment

• Document services delivered to patients in the electronic health record and maintain accurate data for required reporting, best practices, and outcome dissemination

• Contribute to institutional policy change to support routine screening and linkage to care

• Demonstrate culturally sensitive services and function effectively within a team-based, inter‑professional approach while possessing strong knowledge of local resources
