Position Summary The RN Admissions Flex is responsible for facilitating timely and effective hospice admissions, ensuring patients and families receive compassionate, informative, and seamless care. This role requires collaboration with healthcare professionals, internal teams, and community partners to identify and overcome barriers to admission, while maintaining compliance with regulatory standards and best practices in hospice care. The RN ensures accurate assessments, develops initial care plans, educates stakeholders on hospice philosophy, and contributes to patient transitions and care coordination. Flexibility is required to work across hospital, long term care, adult living facilities and home settings.

Employees assigned this job title work understand that the length of the workday is determined by workload. Employees must be able to flex hours as needed to meet referral and admission requests.

Essential Functions

Promote hospice services and philosophy to patients, families, healthcare professionals, and the broader community.

Evaluate hospice eligibility using Medicare Local Coverage Determination and clinical judgment.

Perform and document comprehensive patient assessments, including physical, psychosocial, spiritual, and functional needs.

Facilitate admission process, including physician order acquisition and communication.

Provide emotional support and education to patients, families, and caregivers.

Collaborate with physicians, referral sources, and interdisciplinary team members to ensure seamless patient care and transition.

Facilitates transfers from current level of care to next level of care based on Medicare Conditions of Participation. Acts as the primary nurse designee while patient is physically in the hospital and communicates all changes as a member of the interdisciplinary care team including updating the patient’s plan of care.

Serve as primary nurse designee for hospital-based patients, updating care plans and coordinating changes.

Maintain confidentiality in accordance with HIPAA regulations.

Educate medical community and partners on hospice referrals and benefits of early referral.

Mentor and train new staff; provide performance feedback to leadership.

Maintain professional representation of hospice organization in all external and internal engagements.

Participate in hospital committees, public relations activities, and educational programs.

Uphold organizational policies and procedures, fiscal responsibility, and ethical nursing standards.

Perform other duties as assigned.

Qualifications Knowledge and Critical Skills

Hospice philosophy, services, and admission criteria

Advance Directives and state-specific regulations

Medicare Conditions of Participation and insurance-related hospice coverage

Pain and symptom management

Community and LTC referral resources

Excellent verbal/written communication and customer service skills

Compassionate communication and cultural sensitivity

Critical thinking, problem-solving, and independent decision-making

Clinical nursing skills and interdisciplinary collaboration

Ability to supervise and mentor staff

Proficiency in EMR systems and Windows-based applications

Experience

Minimum of 2 years in hospital or long-term care settings

ICU or ER experience preferred

Prior hospice experience a plus

License / Certification

Current Registered Nurse license in the State of Florida.

Current CPR certification.

Valid driver’s license and acceptable driving record.

Certification in Hospice and Palliative Nursing (preferred).

Education

Diploma in Nursing, Associate Degree in Nursing (ADN), or Bachelor of Science in Nursing (BSN) from an accredited program.

Other Requirements

Have reliable transportation and flexibility to work across multiple locations.

Meet OSHA and CMS immunization and safety requirements including medical clearance and up-to-date vaccinations (TB, Hepatitis, Flu)
