Position Overview
Pre-Enrollment Support Coordinator, Career Service. Pay: $34,760 to $38,264 annually. Closing date: 01/23/2026.
Responsibilities
Track and coordinate the Crisis Waiver Enrollment (CWE) requests and update client records in APD iConnect.
Obtain missing supporting documentation and provide technical assistance to consumers and legal representatives during the CWE review process.
Identify immediate local, state, and community resources to alleviate critical needs of consumers and families during CWE review.
Issue written notices of crisis denial or approval upon completion of CWE process.
Facilitate the iBudget waiver enrollment process for approved Pre‑Enrollment clients and complete closure procedures per existing rules.
Create and update short form support plans for clients upon initial placement and annually thereafter.
Advocate for Pre‑Enrollment clients and families, document contacts and case‑management activities in APD iConnect.
Conduct disaster planning and quarterly review activities for clients in Supported Living.
Coordinate delivery of APD funded supports and services through the Individual and Family Supports (IFS) program and ensure invoices are authorized and prepared within five working days.
Complete other duties and projects as assigned.
Minimum Qualifications
High school diploma or equivalent.
Valid driver’s license and reliable transportation.
At least two years of professional experience with developmental disabilities or mental illness, or allied health/rehabilitative sciences principles and practices.
Case‑management experience.
Customer‑service experience.
Preference for bilingual (English & Spanish) applicants.
Knowledge, Skills, and Abilities Knowledge of:
databases, electronic files, Microsoft Office, case‑management best practices.
Ability to:
plan, organize, and coordinate work assignments; actively listen; communicate effectively verbally and in writing; establish and maintain working relationships; solve problems and make decisions; manage a complex caseload; represent programs and policies assertively; work independently and as part of a team; travel for work; prioritize and schedule work efficiently; multi‑task under pressure.
Demonstrated skills in:
case management; excellent customer service.
