Position Summary This responsible professional position provides support coordination to consumers with special needs arising from developmental disability. Employees maintain client records in APD iConnect, perform detailed reviews of multiple documents, and complete tasks related to the Pre‑Enrollment Workstream with limited supervision.
The Work You Will Do
Track and coordinate Crisis Waiver Enrollment requests; update client records in APD iConnect, including the Crisis Identification Tool, Programs, Divisions, and Notes Tabs.
Work with families and agencies to obtain missing supporting documentation for CWE requests.
Provide technical assistance to consumers and legal representatives regarding the CWE review process.
Identify local, state, and community resources to address immediate critical needs while the CWE process is underway.
Issue written notices of crisis denial or approval upon completion of the CWE process.
Facilitate iBudget waiver enrollment for approved Pre‑Enrollment clients, complete closure procedures for clients meeting closure requirements, and maintain records per the iConnect Case Management Manual.
Create and update support plans and Annual Status Review Forms.
Advocate on behalf of Pre‑Enrollment clients and families; document contacts and case management activities in APD iConnect.
Conduct disaster planning and quarterly review activities for Pre‑Enrollment clients in Supported Living.
Coordinate delivery of APD funded Supports and Services through the Individual and Family Supports program, ensuring appropriate service delivery and authorization of invoices.
Complete other duties and projects as assigned.
Minimum Qualifications
High school diploma or equivalent.
Valid driver’s license or efficient means of transportation.
At least two years of professional work experience with developmental disabilities, mental illness, allied health, or rehabilitative sciences.
Case management experience.
Customer service experience.
Preference for candidates who are bilingual (English & Spanish).
Knowledge, Skills, and Abilities
Proficient in databases, electronic files, and Microsoft Office.
Case‑management best practices and ability to manage a complex caseload.
Strong communication, listening, problem‑solving, and decision‑making skills.
Ability to plan, organize, prioritize, and coordinate work assignments independently or as part of a team.
Travel for work purposes.
Capacity to work under pressure and multi‑task.
