Statement of the job Under the direction of the Medical Director and team of Nurse Practitioners, the Physician Assistant will support primary care screening and monitoring services to patients. The Physician Assistant will have regular interactions with key program staff to ensure coordinated, whole-person care on behalf of CCNS’ clients, the vast majority of whom are living with mental health and/or substance use conditions. The Physician Assistant will consult with the Medical Director and refer individuals out for any additional primary care or specialty medical services needed.
Under the direction of the Medical Director, perform primary care screenings and assessments, including gathering information from clients about their medical history and physical health symptoms.
In close collaboration with CCNS’ team of Nurse Practitioners, provide the allowable primary care screening and monitoring services, including:
Health Physicals (New or Established Patients)
OMH Health Monitoring (15 min, 30 min, 45 min, 60 min)
OMH Health Monitoring Group (30 min or 60 min)
OMH or OASAS Smoking Cessation Treatment for an Individual (3-10 min or > 10 min)
OMH Smoking Cessation Group (>10 min)
Work onsite at CCNS’ program to provide clients with outpatient primary care screening and monitoring of key health indicators and health risks.
Work closely with the Medical Director, Nurse Practitioners, and other staff to ensure that all clients are provided screenings related to the following conditions and health risk factors:
HIV and Viral Hepatitis screening
Basic vitals, medical history, and current physical health symptoms
Body Mass Index (BMI), BMI percentile (for children and youth) and Nutrition/Exercise levels
Chronic diseases screening
As needed, work with the Medical Director and the Team to order diagnostic testing (e.g., bloodwork at an external lab) and make referrals to external primary care providers to address more intensive primary care or specialty medical needs.
Support the identification and ongoing monitoring of individuals served who are living with chronic health conditions.
Closely collaborate with CCNS’ integrated care team to provide whole-person services (addressing everyone’s physical, mental health, and social needs), adjusting client services/treatment plans when needed to support overall wellness and recovery.
Perform other duties as directed.
Qualifications
NYS Licensed Physician Assistant
Certification from National Commission on Certification of Physician Assistants (NCCPA)
At least 1 year experience providing primary care services to individuals living with mental health conditions and/or substance use disorders.
Medicaid Provider Number required
Have successfully completed a NYS-accredited Physician Assistance program.
Proficiency with statistical software and using electronic health records.
Experience supporting programs using the Netsmart myEvolv electronic health record preferred.
Knowledge of the CCBHC Demonstration program model and evaluation requirements preferred.
Knowledge of NYSOMH and OASAS regulations/requirements preferred.
Able to work flexible hours and days -including weekends/evenings/holidays when requested, according to needs of a 24/7 programs.
Regularly required to talk, hear, walk, stand, & sit.
Frequently lifts and/or moves up to 10 pounds.
