We are seeking a Care Coordinator to join our behavioral health clinic and support integrated care delivery across our service catchment area. This role works collaboratively with a Behavioral Health team to assist individuals in accessing care, navigating the healthcare system, and ensuring high-quality outcomes through monitoring clinical measures and recommending improvement strategies.

Three years of experience in administrative services; OR
Six years of full‑time work experience in any field; OR
A bachelor's degree.
EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full‑time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field. Job Duties
Serves as a Care Coordinator team member working directly with Behavioral Health and Primary care Management teams, which provides care coordination services to the entire service catchment area.
Independently assist persons served with access to care, help persons served navigate the complex health care system, and to ensure the quality of their care by monitoring clinical outcome measures and developing recommendations related to improvement measures.
Works alongside a multidisciplinary healthcare team (which may include the Medical Director, Psychiatrists, Medical Psychologists, Psychiatric Mental Health Nurse Practitioners, Primary Care Nurse Practitioners, RNs, LPNs, pharmacists, and clinicians) in order to ensure coordination of care and appropriate communication of care among multiple disciplines.
Identify and manage clients’ chronic conditions and needs, including behavioral health, preventive health, and clinical measures to be implemented.
The coordination of care will include face‑to‑face, virtual, and telephone contact with clients, family, caregivers, physical care team, specialty care team, and other providers and involves secure sharing of information to promote safe and effective care.
Position‑Specific Details Appointment Type:
Probational Appointment. Applicants may be asked to serve a probationary period that may be anywhere between 6 months and 24 months.
Compensation:
The salary offered will be determined based on qualifications and experience.
