Primary Responsibilities Encourage clients and their families to navigate and access community services, adopt healthy behaviors, promote and maintain holistic health (physical, emotional, spiritual). Provide social support, informal counseling, advocacy for clients with providers and community supports, outreach, short‑term case management services. Fulfill all contractual requirements for grant funding and meet outcome goals.

Minimum Knowledge, Experience, and Skills Required “Embrace the mission and vision of the Catholic Diocese of Green Bay and work in accordance with Catholic Teachings and the moral and ethical values of the Catholic Church.”

Associate degree or equivalent based on relevant work experience.

Bilingual or multilingual preferred (written and oral proficiency).

Preferred: prior experience working in a multicultural setting and/or community health worker certification.

Ideal candidate: bachelor’s degree with knowledge in social work, human development, community services, psychology, human services, community health, or related field.

Experience working in a community‑based setting for at least 1–2 years preferred.

Proficient in Microsoft Office (Word, Excel, PowerPoint, Teams, Outlook).

Excellent writing and interpersonal skills.

Strong communication skills.

Ability to work in a team environment.

Ability and willingness to provide emotional support, encouragement, and motivation to clients so they can navigate systems independently.

Skilled in problem solving and finding creative solutions to challenges.

Ability to work with other systems without intimidation.

Must be able to organize and prioritize work, take initiative, follow through and manage multiple priorities to ensure work and goals are met in a timely manner.
