Department of Aging Human Services Associate Community Outreach Specialist
Under general supervision, this role provides case management services to older adults residing in Baltimore County. The position is classified as Pay Schedule I, Grade 25, with a regular schedule of 35 hours per week.
Job Purpose To assess, plan, and coordinate services for older adults, ensuring timely access to benefits, support services, and appropriate care plans.
Essential Job Duties
Interviews clients to establish case history or review case histories of referrals in order to assess needs.
Schedules an appointment for a home visit or office visit.
Discusses possible resources or interventions with clients to address the situation and jointly create an action plan.
Assists clients in applying for benefits.
Arranges and monitors services for grant‑funded clients, including personal care services, medication supplies, adult day services, and related services.
Intervenes in individual client crisis situations, including utility shutdowns, evictions, and lack of funds for medication or food.
Maintains client records in electronic format via WellSky.
Conducts educational, community relations, prevention, and outreach programs.
Attends meetings and training sessions.
Maintains expertise in the field of aging, resources for older adults, and public benefits.
Examples of Other Duties Performs other duties as assigned.
Qualifications Required Qualifications
Bachelor’s degree from an accredited college or university with a major in social or behavioral science.
Experience as described in the essential duties may be substituted on a year‑for‑year basis, up to a maximum of four years, for the required education.
Knowledge, Skills and Abilities
Knowledge of theories and principles of aging.
Basic knowledge of community, human, and social services assistance programs.
Writing skills necessary to complete forms, case histories, and reports.
Good interpersonal skills and active listening skills.
Ability to conduct individual diagnostic/assessment interviews and to formulate a basic plan of care.
Ability to make appropriate referrals.
Ability to establish and maintain good working relationships with others.
Ability to appropriately handle hostile individuals and those experiencing a crisis.
Ability to enter and maintain client information in electronic records.
Ability to make public presentations.
Ability to interpret federal, state, and local regulations, procedures, and guidelines for human services professionals.
Ability to read and understand professional journals and other publications.
Licenses and Certificates Some positions within this classification may require possession of a valid driver’s license equivalent to a non‑commercial, class C Maryland driver’s license at the time of appointment.
Proof of Licenses, Certifications and Education Applicants are required to submit proof of education, professional license(s), and certification(s). Diplomas or transcripts must show the applicant’s major field of study. Copies and unofficial transcripts are acceptable.
