Job Description Are you an experienced
LVN Service Coordinato r looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered “yes" to any of these questions – this is the position for you!
Daily Responsibilities:
Will be completing “2060 Assessments,” assessing patients’ ADLs (Activities of Daily Living) and need for services
Talking on the phone with the ABD (aged, blind, disabled) population
Hours for this Position:
Mon-Fri 8:00-5:00, possibility of OT and/or Saturdays

Competitive salary, negotiable based on relevant experience ($19-$23/hr.)

Qualifications
LTC or Home Health experience (approx. 2 years)
Look for experience working with the Aged, Blind and Disabled population (SSI patients on disability)
Experience assessing ADLs (have used the 2060 form)
