Job Description Are you an experienced RN Service Coordinator looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered “yes" to any of these questions – this is the position for you!

Daily Responsibilities

Collaborate with medical, behavioral, and social support staff in conducting screening and risk assessment interventions based on program guidelines

Identify special needs members through the completion of health screens and other resources, and advise, assist and authorize appropriate services

Review and evaluate all requests for specialized and private care including nursing services

Act as a clinical resource to complex medical cases and individual service plans

Act as a liaison between members, providers and pharmacies and review and follow-up on medical referral calls

Hours for this Position

Pay rate starts at $29 per hour

Qualifications

Experience working with the Aged, Blind and Disabled population (SSI patients on disability)

Experience coordinating services and knowledge of community/government-funded services in the area

Strong candidate - experience coordinating services, assessing ADLs and evaluating what is needed to keep patient living safely at home/out of hospital
