Locums (Potential Perm) Hybrid Nurse Practitioner or Physician Assistant

We are seeking an experiencedNursePractitioner (NP) or Physician Assistant (PA)for aLocums assignmentwith potential to convert to Permat a primary care practice servinggeriatric patients in avalue-based care model. This is a hybrid role with blend of coverage in outpatient clinics as well as home-based patient care.
Compensation: $75 – $85 per hour
Position Details:
Schedule: Monday – Friday (2–5 days perweek; flexible days that suit your availability)
Patient Load:
Home Care: 3–5 patients per day
Clinic: Small census of 7–10 patients perday
Setting: Hybrid (Outpatient clinic +home-based care), value-based primary care for elderly patients
Responsibilities:
Deliver comprehensive primary care services,including preventive care, chronic disease management, and acute care
Diagnose, treat, prescribe medications, anddevelop individualized treatment plans
Order and interpret diagnostic tests (labs,imaging, etc.)
Document accurately in the EHR system
Provide patient education and promotewellness initiatives
Collaborate with physicians, nurses, andsupport staff to ensure coordinated, high-quality care
Refer patients to specialists and managefollow-ups
Ensure compliance with state, federal, andprofessional regulations
Requirements:
Active NP or PA license in NY
Minimum 2 years of primary care experience
National Board Certification
Bilingual a plus
Medicaid and Medicare Provider Id required
