Position Overview We are seeking a motivated and collaborative Physical Therapist (PT) to support children across outpatient clinic, home-based, and community settings. In this role, you will advocate for each child’s functional needs while delivering evidence-based interventions and partnering with families and multidisciplinary team members to maximize developmental outcomes. This position offers flexibility, variety in service settings, and the opportunity to make a meaningful impact across the continuum of care.

Key Responsibilities Provide evaluations and evidence-based PT interventions in clinic, home, and community settings Develop individualized treatment plans targeting strength, balance, mobility, and functional skills Monitor, document, and report patient progress in compliance with documentation and billing standards Collaborate closely with SLPs, OTs, family therapists, and other team members to ensure coordinated care Coach and empower families to carry over therapeutic strategies into daily routines Participate in team meetings, IFSP/IEP reviews, 6-month reviews, and program benchmarks Supervise Physical Therapist Assistants (PTAs) as required

Qualifications Graduate degree in Physical Therapy Active New Mexico Physical Therapy license Strong organizational, communication, and interpersonal skills Ability to work independently while contributing to a team-based care model Valid driver’s license and reliable transportation for community and home-based services Relocation assistance available for qualified candidates

Physical Requirements This role requires the ability to lift up to 40 lbs and perform physical activities such as standing, walking, bending, kneeling, and navigating varied environments. A valid driver’s license and reliable vehicle are required for home and community visits.
