$73,019.12-$109,528.97 Sign-On Bonus:

$5,000 Job Description: The Care Management Social Worker provides social work services to patients and families, including assessing needs, formulating treatment plans, and coordinating care with the interdisciplinary team. They offer counseling, crisis intervention, and advocacy services, ensuring patients receive comprehensive care and support. The role involves collaborating with community agencies, leading educational groups, and maintaining documentation standards. The ideal candidate holds a Master of Social Work Degree, is a licensed social worker (LMSW/LCSW) in New York, and has at least two years of experience in an acute care hospital setting or related environment. NYS SCREEN Certification is required within the initial 3 months of employment. Responsibilities: Conduct interviews with patients and families to determine continuing care needs Provide psychosocial information for patient transfers Offer counseling, crisis intervention, and grief counseling Assist patients with adjustment to treatment Devise and implement social work treatment plans Provide information, referral, and advocacy to community agencies Lead education and support groups Identify, evaluate, and design safety plans for victims of crimes/abuse Maintain and complete statistics of case activities Attend meetings and participate in presentations, training, and in-services as required Assist/participate in presentations, training, and in-services as required Participate in the department's cross-training program Respond to internal and external disasters when indicated/assigned by supervisor Perform other duties as assigned Requirements: Master of Social Work Degree from a Council of Social Work Education accredited School of Social Work Licensed Social Worker (LMSW/LCSW) approved by the State Education Department of the University of the State of New York Two years experience in an acute care hospital setting or related setting NYS SCREEN Certification (obtained within initial 3 months of employment) Ability to relate cooperatively and constructively with patients, families, significant others, co-workers, physicians, and other staff members Physical/Mental Demands/Requirements & Work Environment: May be exposed to chemicals necessary to perform required tasks Must be physically able to write, read, and move quickly throughout the work environment and hospital Must have the dexterity to operate computers and other office equipment Must have the ability to assess a patient's situation, formulate a plan according to need, and follow through to resolution Must be able to communicate with patients and others in a sensitive and caring manner and to visually assess a patient's affect and body language Note : The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
