Primary Care Physician (PCP) in our organization demonstrates:

Accountability for outcomes: strong clinical care and cost‑effectiveness for each patient in a panel of up to 450 patients.

Coaching for health: acting as a health coach rather than a consultant, helping patients set short‑ and long‑term health goals and frequently following up.

Simplifying for action: simplifying and prioritizing care to make behavior change actionable for patients and the team.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

Provide independent care for patients with acute and chronic illnesses in older adults.

Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining involved in the detailed care.

Engage with the hospitalist for any patient in a hospital (regardless of employment).

Lead assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for complex chronic health needs.

Lead the care team comprising a care promoter (medical assistant), care facilitator, and care coordinator for office patients.

For patients unable to come to the office—Hospital, SNF, LTC, or homebound—engage with the transitional care team and other resources available.

Assume an active role in center management, cover for other providers, and participate in recruiting patients and additional providers.

Perform other duties as assigned or modified by the manager.

KNOWLEDGE, SKILLS & ABILITIES:

Availability and accessibility: open and available for walk‑in visits and timely response to calls and messages.

Service orientation: provide care that you would want for a family member at each interaction.

Evidence‑based medicine: stay updated on clinical, nutritional, and lifestyle interventions.

Physician leadership: continuously develop and improve leadership skills for patients, team, center, and company.

Quality: pursue high‑quality outcomes and systematically improve practice and operations.

Influence: competently and compassionately influence patients and teams.

Self‑care: maintain physical, mental, social, and spiritual well‑being to care effectively for patients.

Proficiency in Microsoft Office Suite and related software.

Ability and willingness to travel locally, nationally for initial training, and occasionally regionally for recruiting or training.

Fluency in English (verbal and written); other language fluency may be required in some centers.

Independent judgment and decision‑making.

EDUCATION AND EXPERIENCE CRITERIA:

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.

State medical license or ability to obtain one in the working state(s).

Board certification (or eligibility) in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.

Maintenance of board certification through MOC, CME, or re‑examination.

Current DEA number for schedule II–V controlled substances.

Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within the first 90 days of employment.

PAY RANGE: $107,903 – $154,147 Salary
