ESSENTIAL JOB DUTIES/RESPONSIBILITIES

The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.

The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, while remaining involved in the patient’s detailed care.

It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital, regardless of hospital affiliation.

The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.

The PCP leads their care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.

For patients unable to come to the office—whether in hospital, SNF, LTC, or homebound—the PCP will engage with the transitional care team and other resources such as case managers, acute and transitional‑care physicians, and others available in the market.

The PCP will have an active role in the management of their center and will help cover for other providers, assist in recruiting patients for the center, and recruit additional providers for the company.

The PCP supervises, collaborates with, and functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP), remaining accountable for the APP’s actions while employed with the company.

Performs other duties as assigned and modified at the manager’s discretion.

KNOWLEDGE, SKILLS & ABILITIES

Availability and accessibility: make themselves available to patients as possible, including walk‑in visits and timely response to calls and messages.

Service orientation: provide care patients would want for a family member or themselves at every interaction.

Evidence‑based medicine: stay updated on medicine, lifestyle, nutrition, and clinical interventions to improve outcomes.

Physician leadership: continuously develop and improve leadership skills for patients, team, center, and company.

Quality focus: work with care teams to achieve high quality outcomes and measures, and continuously improve practice operations.

Influence: competently and compassionately influence patients, teams, and themselves to attain the best outcomes.

Self‑care: maintain personal physical, mental, social, and spiritual well‑being to care for patients effectively.

Proficient with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other software used in the company.

Willingness to travel locally as needed, nationally for initial 2‑week training, and occasionally regionally and nationally for recruiting or training.

Fluency in English (verbal and written); fluency in other languages may be required for some centers.

Independent judgment in clinical decision‑making.

Flexibility to work evenings, weekends, and/or holidays as needed.

EDUCATION AND EXPERIENCE CRITERIA

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.

State Medical License must be obtained or already active for the state(s) of practice.

Board certification in the specialty preferred; board eligibility required.

Maintain board certification through MOC, CME, or retaking exams as required.

Current DEA number for schedule II‑V controlled substances.

Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days.

PAY RANGE $214,700 - $306,714 Salary
