Job Description:
We’re seeking a compassionate and detail-oriented Registered Nurse (RN) Case Manager to join our team!
Utilize a collaborative process of assessment, planning, facilitation, and advocacy to help members achieve optimal, cost‑effective outcomes.
Work closely with members, providers, and internal teams to ensure the right care at the right time.
Travel to SW Michigan Area counties listed above.
Key Responsibilities
Conduct comprehensive assessments of referred members’ needs and eligibility.
Evaluate members’ benefit plans and determine appropriate case management approaches.
Apply and interpret clinical criteria, guidelines, and policies to ensure benefits are administered appropriately.
Utilize case management and quality management processes in compliance with company, regulatory, and accreditation standards.
Coordinate internal and external resources to meet member needs effectively.
Qualifications
Active, unrestricted RN license (required).
Case Management Certification (CCM) preferred.
Minimum of 2–3 years of clinical experience (hospital, home health, or ambulatory care).
Healthcare and/or managed care industry experience required.
Case management experience strongly preferred.
