Base pay range $97,457.00/yr - $159,428.00/yr
Summary This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within 4 months of appointment. Program approval – award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants are ineligible to apply.
Responsibilities
Independently manage a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence‑based practice.
Develop and implement an individualized plan to achieve optimal population health and disease management.
Demonstrate leadership and work collaboratively with the interdisciplinary team.
Order and interpret labs, tests, and diagnostic studies.
Prescribe pharmacologic and non‑pharmacologic interventions.
Initiate referrals and consults to specialty services.
Provide education to patients and their families.
Assess and adjust the plan of care as needed to achieve optimal patient outcomes.

Work schedule: 8:00 am – 4:30 pm Monday‑Friday – subject to change based on facility needs.

Relocation/Recruitment Incentives: Not authorized. EDRP authorized. Former EDRP participants are ineligible for incentive.
