Licensed Practical Nurse (LPN) – ACT Team Nurse
Join Clarvida as an ACT (Assertive Community Treatment) nurse and become a critical member of a multidisciplinary team caring for adults with severe mental illness. You will provide medical treatment and support services in clients’ homes, community settings, and the office. Responsibilities
Meet clients in their homes, community, and office (including Zoom/phone). Perform basic vitals and labs. Administer long‑acting injectable medications. Provide medication‑management support and coordinate care with medical providers. Transport clients using the company car as needed. Participate in a one‑week on‑call rotation every 7‑10 weeks. On‑call may be handled virtually or in person. Qualifications
LPN license in Louisiana. Experience treating adults with severe and persistent mental illness and/or substance‑abuse issues. Psychiatric nursing experience preferred. Valid driver’s license in good standing and current auto insurance. Perks
Hourly pay: $23.07–$24.50. Standard weekday schedule (8 a.m.–5 p.m.) with flexible hours. Growth and stability working within a national agency. Benefits
