Class LTD is seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a critical role in supporting individuals and families by providing comprehensive case management services. This position involves assessing client needs, developing tailored service plans, and coordinating resources to ensure clients receive the necessary support for their well‑being and success.
Duties
Conduct thorough assessments of clients' needs, strengths, and challenges to develop individualized service plans.
Coordinate with various agencies and service providers to facilitate access to resources such as housing, healthcare, and employment opportunities.
Provide crisis intervention and support during critical situations, ensuring clients receive immediate assistance.
Monitor client progress and adjust service plans as necessary to meet evolving needs.
Offer guidance in areas such as workforce development, addiction counseling, child welfare, and behavioral health.
Supervise and mentor junior staff or interns as needed to enhance team effectiveness.
Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies.
Advocate for clients' rights and empower them through education about available services and resources.
Requirements
Bachelor’s degree in Social Work, Psychology, or a related field is preferred; relevant experience may be considered in lieu of a degree.
Experience in case management or related fields such as childcare or behavioral health is highly desirable.
Strong skills in crisis intervention and conflict resolution are essential for success in this role.
Familiarity with Section 8 housing programs is a plus.
Excellent communication skills, both verbal and written, to effectively interact with clients and stakeholders.
Ability to work collaboratively within a multidisciplinary team while also demonstrating strong independent decision‑making capabilities.
