Case Manager
to join our team. Asteroid Health is a modern outpatient mental health program offering OP, IOP, and PHP services. The
Case
Manager provides comprehensive, client-centered case management to adults with mental health needs, ensuring they are connected to the right care, at the right time, with the right supports.
Duties Conduct comprehensive assessments to identify client needs related to behavioral health, addiction, child welfare, and other social services. Develop individualized care plans tailored to each client's circumstances and goals. Provide crisis intervention and crisis management support to clients experiencing urgent situations. Utilize motivational interviewing techniques to encourage positive behavioral changes. Coordinate with community resources such as Section 8 housing, Medicare services, workforce development programs, and childcare providers. Monitor client progress and adjust care plans as needed to ensure effective outcomes. Offer mentoring and supportive guidance to clients, fostering independence and resilience. Supervise or collaborate with other staff or volunteers involved in client care when applicable. Maintain accurate documentation of all interactions, assessments, and service plans in compliance with organizational policies.
Qualifications Proven experience in social work, behavioral health, or related fields such as child welfare or workforce development. Strong skills in crisis intervention, crisis management, addiction counseling, and motivational interviewing. Supervising experience is preferred but not required; leadership qualities are a plus. Knowledge of Section 8 housing programs, Medicare services, and child welfare systems highly desirable. Excellent communication and interpersonal skills with the ability to build rapport with diverse populations. Compassionate approach with a commitment to client-centered care. Ability to handle sensitive situations discreetly and professionally. Relevant certifications or licenses in social work or counseling are advantageous but not mandatory.
