As a Registered Nurse for Admission/Discharge, you deliver patient‑centered care, ensuring efficient transitions for patients entering and leaving acute‑care settings. You perform specialized assessment, maintain documentation, educate patients/families, coordinate with administrative staff, and advocate for optimal clinical outcomes. Responsibilities

Obtain a nursing assessment history and ensure proper inpatient admission; communicate special circumstances to physicians and care team. Complete required documentation (admission assessments, risk assessments, medication reconciliation, discharge paperwork, post‑hospital documentation). Teach patients/families/caregivers about the patient’s medical condition, status, treatment plan, medications, side effects, and follow‑up measures; translate complex terminology. Complete extra discharge documentation, education, and discharge care for special patient populations to comply with core measures. Ensure patients/families/caregivers receive copies of discharge paperwork and other documentation; provide referrals for ongoing care and educational materials. Coordinate with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas for smooth transitions. Demonstrate a time‑conscious awareness and ability to prioritize; distinguish between low and high‑priority activities. Qualifications

Advanced Cardiac Life Support (ACLS) certification within 6 months of employment start date. Basic Cardiac Life Support (BCLS) certification within 30 days of employment start date. (RN) Registered Nurse certification within 30 days of employment start date. Associate Degree, Bachelor’s Degree, or RN Diploma. Acute care experience with cardiac monitoring required.
