Physician — PRN, Hampton, VA

Key Responsibilities
Perform comprehensive assessments and develop individualized care plans for residents in post‑acute and long‑term care settings
Diagnose, treat and manage acute and chronic medical conditions consistent with current evidence‑based practice
Provide medication management, including review of polypharmacy and coordination of medication reconciliation
Respond promptly to acute changes in condition, initiate timely interventions and coordinate transfers of care when necessary
Collaborate with nursing staff, advanced practice clinicians, therapists, social workers and facility leadership to deliver holistic, person‑centered care
Participate in facility rounds, care‑planning meetings and interdisciplinary case conferences
Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)
Support medication safety initiatives, including antimicrobial and antipsychotic stewardship
Provide clinical consultation and education to facility staff and medical colleagues on complex clinical issues and best practices
Maintain professional standard of practice, including infection control, documentation and regulatory compliance
Essential Qualifications
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited institution
Active and unrestricted Virginia medical license
Board certification or eligibility in an appropriate specialty (family medicine, internal medicine, geriatrics) preferred
Clinical experience in post‑acute care, long‑term care, geriatrics or related settings preferred
Proficiency with Electronic Medical Record systems and clinical documentation
Strong clinical judgement, interpersonal and communication skills; ability to work effectively within multidisciplinary teams
Commitment to patient‑centered care, cultural sensitivity and ethical practice
Current Basic Life Support (BLS) certification; Advanced Cardiac Life Support (ACLS) preferred where applicable
