Job Summary : To contribute to the success of the community by providing quality nursing care to residents and to coordinate total nursing care for residents by implementing specific procedures/programs and being in compliance with all STGi policies, state regulations and federal regulations.

Essential Functions

Follow established standards of nursing practices and implement STGi programs, policies and procedures.

Supervise and evaluate all direct resident care and initiate corrective action as necessary.

Obtain report from nurse being relieved and record sufficient information to implement appropriate follow‑up action as necessary.

Provide report to nurse coming on duty, including sufficient information for follow‑up action as necessary.

Take appropriate action to provide staffing to meet residents’ needs.

Conduct resident rounds daily, reporting problems to nursing supervisor and initiating corrective action.

Identify safety hazards and initiate corrective action immediately.

Identify resident problems and emergency situations and initiate emergency care per physician’s orders in the absence of a physician.

Provide supervision to Nursing Assistants providing direct resident care.

Attend staff meetings, interdisciplinary team meetings, rehabilitation meetings, in‑service classes and committee meetings as assigned.

Review the Glucometer Control Log to monitor the machine was checked that day prior to testing for blood sugar levels.

Follow staffing guidelines and make nursing assignments according to qualifications of staff and characteristics of the resident population.

Counsel Nursing Assistants and recommend disciplinary action to the Director of Nursing Service when necessary.

Comply with community processes related to pain assessment/management/documentation, providing treatments/documentation and delivering medications/documentation.

Administer and document direct resident care, medications and treatments per physicians’ orders and accurately record all care provided.

Provide instruction for appropriate nursing care to licensed nurses and Nursing Assistants.

Document assessment of residents as assigned as per licensure. Report untoward findings to physician and supervisor.

Observe the 5 RIGHTS of medication/treatment administration.

Promptly report changes in residents’ conditions to the physician, Director of Nursing Service and responsible party, and take follow‑up action as necessary. Document notifications in resident’s clinical record.

Follow Community admission procedures and communicate resident admission to other departments, including rehabilitation.

Competently perform basic nursing skills as per licensure.

Follow Community discharge /transfer procedure/documentation and coordinate discharge/transfer with other departments, physicians and ancillary service providers.

Supervise response to resident’s call for assistance.

Listen to resident, family, or physician concerns and initiate corrective action and/or report concerns to Nursing Supervisor.

Count narcotics, check emergency kit and exchange keys each shift.

Check meal trays and nourishment to monitor proper diet is served and residents are assisted as necessary.

Order, receive and store medications appropriately.

Observe community process related to signing out narcotic and administration thereof. This includes the process related to shift narcotic count and documentation thereof.

Notify physician, when necessary, of changes in residents’ conditions, weight, medication errors, medication not available, abnormal laboratory/X‑Ray results and any other unusual observations.

Document daily notes on unstable residents/change in resident condition, Medicare residents and recently admitted residents.

Be responsible for the safety of residents and team members under supervision.

Observe infection control procedures.

Perform all the duties of a certified nursing assistant when required.

Required Skills & Experience Must have a current and valid RN nursing license in the state of Texas, or a current and valid RN license from a Compact Party State.

Required Education Current Basic Life Support (BLS)/Cardiopulmonary Resuscitation (CPR) certification required prior to performing work, preferably from American Heart Association (AHA), or the ability to obtain certification within thirty (30) days of employment.

Working Conditions / Physical Requirements

This position primarily involves standing, walking, twisting, reaching, bending/stooping, pushing and pulling (typical weight of 5 lbs. and maximum weight of 30 lbs., approximately), and lifting (minimum weight of 5 lbs. and maximum weight of 100 lbs., approximately).

The position requires repetitive finger movements as well as hearing and talking on the phone and in person.

Must be able to continuously deal effectively with stress created by residents, multiple tasks, noises, interruptions, and work cooperatively as part of the health care team while maintaining a pleasant attitude.
