24/7 Staffing Support
Location Lake City / Cadillac or Traverse City, MI (options). Parent agency located at Charlevoix.
Responsibilities
Provide emotional support to help patients live their final days with dignity and comfort.
Conduct supervisory visits with assigned team members for clinical observation and performance review.
Participate in interdisciplinary group meetings, providing feedback on patient/family care plans.
Assist in the development, review, and revision of the patient/family plan of care.
Prepare clinical and progress notes and participate in IDG conferences.
Participate in Hospice performance improvement program.
Maintain high level of customer satisfaction with clients, families and caregivers.
Communicate information using current processes and technology within the organization.
Qualifications
Registered Nurse with a current Michigan license in good standing.
Minimum 1 year of hospice experience.
Empathetic approach to patients and caregivers.
Initial and ongoing assessment of patient’s physical, functional, psychosocial and environmental needs.
Implement individualized plan of care and revise as needed.
Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance.
Consulting with and educating patient/family on disease process, self‑care techniques, end‑of‑life care, and ethical issues.
Supervision of paraprofessionals providing services to patients.
Participate in field visits daily, weekly office meetings as indicated.
Adapt to patient status and needs on a full‑time on‑call basis.
