Salary & Shift Availability $26–$30/hr

(case dependent).

Available in San Bernardino, Fontana, Riverside, and Rancho Cucamonga.

We offer full‑time, part‑time, and PRN shifts, including nights, weekends, and days – you can work with your availability.

Essential Job Functions

Deliver and coordinate skilled nursing care under the supervision of a Registered Nurse, following physicians’ orders and care plans.

Continuously assess patient condition, documenting changes and notifying the supervisor or physician as needed.

Document all patient interactions, assessments, nursing actions, and care coordination in a timely manner.

Implement, update, and collaborate on the nursing care plan.

Respond to changes in patient needs, implementing care to prevent risk or reduce risk.

Adhere to Aveanna policies, state & federal regulations, accrediting bodies, and the Nurse Practice Act.

Apply infection‑control measures that protect staff and patients in accordance with OSHA standards.

Educate patients and families about disease processes, self‑care, and prevention strategies.

Maintain competency through continuing education and skill evaluations.

Qualifications

Active, unencumbered California LVN license.

Current CPR certification (hands‑on component).

Valid TB skin test (within last 12 months).

At least one year of hands‑on nursing experience.

Reliable transportation.

Proficient in clinical assessment, documentation, and compliance with nursing protocols.

Continuing education as required by the state.

Physical & Environmental Requirements

Speak, write, read, and understand English.

Travel and lift up to 50 lb.

Reposition patients and move equipment without assistance.

Walk, stand, bend, kneel, reach, twist, and climb stairs for prolonged periods.

Maintain visual and hearing acuity; strong sense of smell and touch.

Respond physically and mentally to emergencies in the home or during transport.

Work in varied environments, including exposure to allergens, blood, bodily fluids, and infectious diseases.
