Position Overview The Licensed Practical Nurse (LPN/LVN) provides and documents skilled nursing care under the supervision of a Registered Nurse, following the care plan and physicians orders while maintaining confidentiality and professional boundaries.
Essential Job Functions
Deliver and coordinate quality patient care per physician orders.
Continuously observe and assess patient condition and report changes to supervisor/physician.
Document all activities, assessments, nursing actions, responses and coordination of care promptly.
Participate, implement, and update the nursing care plan.
Take appropriate nursing actions based on assessment to achieve expected outcomes.
Recognize changes in patient needs and intervene to prevent or reduce risk.
Accept responsibility for personal and professional accountability, complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
Use infection control measuresباحية according to OSHA standards.
Educate patient and family on disease process, self‑care, and prevention strategies.
Maintain nursing competence through education, internal learning management, and skill evaluations.
Qualifications
Active, unencumbered CA LVN license.
Current CPR certification with hands‑on component.
TB skin test (two‑step) current within the last 12 months.
One year of prior hands‑on nursing experience.
Reliable transportation.
Continuing education required by state.
Additional State Specific Requirements
South Carolina – One (1) year of tungsten experience.
California – One (1) year of experience under current nursing license.
Louisiana – One (1) year of experience under state license.
Continuing education as required by state.
Preferences
Six (6) months recent experience as a Licensed Nurse in a clinical setting.
Home‑health experience.
Other Skills/Abilities
Attention to detail.
Time management.
Effective problem‑solving and conflict resolution.
Good organization and communication skills.
Physical Requirements
Speak, write, read, and understand English.

Lift 50 pounds and reposition patients without assistance.
