Position Overview The Service Coordinator will be responsible for ensuring that children and families receive timely, high‑quality referrals, evaluations, assessments, and eligibility determinations, supported by accurate and thorough documentation.
Salary / Hourly Rate $20/hr
Responsibilities
Serve as the initial point of contact for children and families, ensuring timely evaluations, eligibility determinations, and accurate documentation in compliance with state and agency standards.
Conduct professional interviews with families and clearly explain rights and confidentiality policies.
Enter referral data, maintain accurate child records, review evaluation paperwork for completeness, and ensure families receive required packets and notices.
Communicate and collaborate with healthcare providers and community agencies to promote coordinated care.
Implement county and regional procedures, manage spreadsheets, and monitor timelines to meet Department of Health criteria for evaluations and transitions.
Facilitate smooth transitions to appropriate service coordination units for eligible children, or provide resources and referrals for families not meeting eligibility, addressing challenges such as mental health concerns, abuse, or developmental disabilities.
Required Experience / Skills
Minimum of one year of documented professional experience working with children and families in a counseling, support, or case‑management role.
Experience preferably with children from birth to five years of age and their families.
Knowledge and experience in non‑profit services, maternal‑child health, and education strongly preferred.
Computer proficiency with Microsoft Windows and Office Suite; familiarity with Excel and Access preferred but not required.
Strong interpersonal skills with excellent oral and written communication abilities.
Valid driver’s license.
Bilingual in English and Spanish.
Education Requirements
Bachelor’s degree in Social Work, Child Development, Education, Psychology, or a related field.
