Position: Nurse Practitioner – Greenville, MS Monogram Health is looking for skilled Nurse Practitioners and Physician Assistants eager to make a difference in patients' lives. The Advanced Practitioner is a key member of an integrated care team including a registered nurse and a social worker. Our patients often struggle with multiple serious diseases. We help them improve quality of life in the home and slow the progression of kidney disease, enabling positive health outcomes.

Your Impact Using your skills, you will deliver personalized compassionate medical care to individuals mainly with CKD and/or ESRD/ESKD. You will be responsible for caring for patients, maintaining accurate records and scheduling, and administering follow‑up appointments as required. Roles And Responsibilities

Conduct assessments, including comprehensive annual wellness exams in patients’ homes and virtually.

Counsel and educate patients and families about benefits and programs to live healthier lives.

Document items such as chief complaint, diagnosis, history, review of systems, exams, medications, allergies, assessment, and plan.

Coordinate care with primary care providers, specialists, and ancillary services.

Complete all documentation and paperwork timely.

Maintain quality of care standards as defined by the practice.

Work remotely in the state of employment and attend periodic training/meetings outside the state.

Deliver evidence‑based, timely care to reduce avoidable hospitalizations and maximize quality of life.

Prescribe medications, order tests, and collaborate with the Monogram physician.

Achieve improved patient quality outcomes, measured and reported daily.

Educate patients about preventive monitoring and work with physician practices to schedule testing.

Assist patients with enrolling to access educational videos.

Participate in integrated care team meetings.

Apply disease diagnosis and prevention knowledge.

Assess patient health status, develop treatment plans, and implement plans consistent with the plan of care.

Follow up and evaluate patient status.

Perform other duties as assigned.

Position Requirements

Bilingual (English/Spanish) highly preferred.

Active and unrestricted RN and NP or PA license.

Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).

Current and unrestricted DEA certificate.

Ability to work autonomously without direct supervision.

Access to transportation, a valid driver’s license, and car insurance.

Proficiency with medical instruments and required equipment.

Knowledge of computer‑based data management programs, information systems, and point‑of‑interview technology.

Excellent verbal and written communication with patients, families, physicians, and community partners.

Sound understanding of federal and state regulations including HIPAA and OSHA.

Minimum two years of direct patient care.

Experience with managed care/IPA/health plan.

Experience conducting annual wellness or similar comprehensive visits virtually or at home.
