Travel Advanced Practice Provider (NP/PA)
Monogram Health is looking for skilled Nurse Practitioners and Physician Assistants eager to make a difference in patients' lives. The Advanced Practitioner is a key member of an integrated Care Team that includes a Registered Nurse and a Social Worker. The patients we serve often struggle with multiple serious diseases.

Salary $95,056.00–$185,848.00
Your Impact As a Travel APP, you will deliver personalized and compassionate care to patients primarily with CKD through ESRD. The position involves traveling throughout the region, connecting with patients in their homes, and providing high‑impact clinical care. Primary duties include patient health assessment, creating strategies to improve or manage a patient's health, and introducing habits for health promotion. You will maintain accurate patient records, schedule appointments, and administer follow‑up care. The role is roughly 4 days a week driving to patient homes and 1 day working telehealth from your home.
Responsibilities
Travel out of town for up to 75% of the month during the work week
Conduct in‑home assessments, including comprehensive annual wellness exams, focused screenings, or other visits both in the patient’s home and virtually
Counsel and educate patients and families about benefits and programs to live healthier lives
Document chief complaints, diagnoses, medical history, medications, allergies, assessment, and plan
Coordinate care with primary providers, specialists, and ancillary services
Complete all documentation and paperwork promptly
Maintain quality of care standards as defined by the practice
Attend periodic training or meetings outside of the state, as needed
Deliver evidence‑based, timely care to reduce avoidable hospitalizations and maximize quality of life
Prescribe medications, order tests, and collaborate with the practicing physician
Measure and report patient quality outcomes daily
Educate patients on preventive monitoring and facilitate diagnostic testing scheduling
Assist patients with enrolling for educational videos
Participate in integrated care team meetings
Apply knowledge of disease diagnosis and prevention
Assess patient health status and implement appropriate plans of care
Follow up and evaluate patient status
Other duties as assigned
Position Requirements
Active and unrestricted Registered Nurse or Nurse Practitioner license
Board certified for appropriate licensure (NP: ANCC/AANP)
Current and unrestricted DEA certificate or eligibility
Willingness to travel extensively across the state
Ability to work autonomously without direct supervision
Valid driver’s license, reliable transportation, and car insurance
Proficiency with medical instruments and equipment required by the work
Knowledge of computer‑based data management programs and medical record systems
Effective verbal and written communication skills with patients, families, physicians, and community partners
Sound understanding of federal and state regulations, including HIPAA and OSHA
Minimum of 2 years of direct patient care experience
