JOB TITLE: Physical Therapist PT - Home Health - Full-time
LOCATON: Waco, TX
RESPONSIBILITIES
Evaluate patients through physical assessments and medical history
Develop personalized treatment plans based on individual needs and goals
Implement therapeutic exercises and training programs
Educate patients and their families on injury prevention and wellness
Monitor and document patients' progress toward rehabilitation goals
Collaborate with other healthcare providers to ensure holistic patient care
Maintain accurate patient records and documentation in compliance with regulations

QULIFICATIONS
Masters degree in physical therapy
1 year of home health experience as a physical therapist
Currently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practices
Must possess and maintain valid CPR certification while employed in a clinical role
Must be a licensed driver who can travel to all business locations.
