Licensed Practical Nurse (LPN) Needed for Adult Patient - Sign on Bonus Aveanna Healthcare is hiring an LPN to provide skilled nursing care for adult patients who are dependent on G‑tube, trach, and ventilator in a home care environment. The role offers a competitive pay range of $27–$30 per hour, a $2,500 sign‑on bonus for full‑time days, and the opportunity to work a flexible schedule that includes weekdays, weekends, and nights.

Position Details

Shift: Sunday through Saturday, 7 AM – 7 PM

Pay Rate: $27–$30/HR

Sign‑on Bonus: $2,500 for full‑time days

Position Overview The Licensed Practical Nurse (LPN/LVN) is responsible for delivering skilled nursing care under the supervision of a registered nurse, following the care plan and physician orders while maintaining confidentiality and professional boundaries.

Essential Job Functions

Deliver and coordinate quality patient care in compliance with physician orders.

Observe and assess patient condition continuously and report changes to supervisor or physician.

Document all activities, assessments, nursing actions, responses, and coordination of care promptly.

Participate in and update nursing care plans.

Take appropriate nursing actions based on assessments to achieve expected outcomes.

Recognize changes in patient needs and intervene to prevent or reduce risk.

Comply with Aveanna policies, state/federal regulations, accrediting bodies, and the Nurse Practice Act.

Provide infection control measures in accordance with OSHA standards.

Educate patient and family about disease processes, self‑care, and prevention strategies.

Maintain professional development through continuing education and internal learning programs.

Qualifications

Active, unencumbered LPN license.

Current CPR certification (hands‑on component).

Reliable transportation.

Proficiency in clinical assessments, documentation, and compliance with nursing policies and procedures.

Continuing education as required by state regulations.

Additional State‑Specific Requirements

South Carolina – One year of pediatrics experience.

California – One year of experience working under a current nursing license.

Louisiana – One year of experience working as a licensed nurse.

Continuing education as required by state.

Preferences

Six months of recent experience as a licensed nurse in a clinical care setting.

Home‑health experience.

Good organization and communication skills.

Physical Requirements

Speak, write, read, and understand English.

Travel as needed.

Lift 50 pounds.

Reposition patients and move equipment without assistance.

Prolonged walking, standing, bending, kneeling, reaching, and twisting.

Ability to sit and climb stairs.

Visual and hearing acuity.

Strong sense of smell and touch.

Respond physically and mentally to emergency situations in the home or during transport.

Environment

Function in various environments, possibly exposing to allergens.

Possible exposure to blood, bodily fluids, and infectious diseases.
