Qualifications

Active, unencumbered

LPN/LVN

license in the state of practice; compact licenses transferred within 90 days

Current CPR certification (hands‑on); Aveanna may assist with certification after hire

Recent TB skin test (within last 12 months)

Six months of prior nursing experience preferred but not required

Reliable transportation

Position Overview The Licensed Practical Nurse is responsible for providing and documenting skilled nursing care under the supervision of a Registered Nurse, in accordance with care plans and physician orders, while adhering to confidentiality standards and professional boundaries.

Essential Job Functions

Deliver and coordinate quality patient care in compliance with physician orders

Observe and assess patient conditions; report changes to supervisor and/or physician

Document all activities, assessments, nursing actions, responses, and coordination of care promptly

Participate in, implement, and update nursing care plans

Take appropriate nursing action based on assessment to achieve expected outcomes

Recognize changes in patient needs, intervene, and implement care to reduce or prevent risk

Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act

Provide care using infection control measures that protect staff and patients per OSHA standards

Educate patients and families regarding disease processes, self‑care techniques, and prevention strategies

Maintain competency through educational programs, continuing education units, and skill evaluations

Requirements

Graduate of an accredited nursing school

Current, unrestricted state license as a Licensed Nurse in the state of practice

Current CPR certification

Proficiency in clinical assessments, documentation, and compliance with nursing care policies

Additional State Specific Requirements

South Carolina – One year of pediatrics experience

California – One year of experience working under a current nursing license

Louisiana – One year of experience working as a licensed nurse

Continuing Education as required by state

Preferences

Six months of recent experience as a Licensed Nurse in a clinical care setting

Home health experience

Speak, write, read, and understand English

Travel as needed

Lift 50 pounds; reposition patients and move equipment without assistance

Visual and hearing acuity; strong sense of smell and touch

Respond physically and mentally to emergency situations in home or during transport

Environment

Work in varied environments, possibly involving allergens and other conditions
