We are looking for a Nurse Practitioner to provide thoughtful, comprehensive palliative care under the supervision of a physician. Your role will involve diagnosing, treating, and caring for patients while supporting families throughout their health journey with compassion and skill.
Responsibilities
Diagnose and treat patients in hospice and palliative care settings.
Provide ongoing medical care, symptom management, and support to patients and families.
Coordinate care with multidisciplinary teams and oversee treatment plans.
Assist families with decision‑making and emotional support.
Maintain accurate patient records and documentation.
Qualifications
Education:
Graduate of an accredited masters, post‑masters, or doctoral nursing program with formal preparation as a Nurse Practitioner.
Licensure:
Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner. Current prescriptive authority with full DEA privileges. Must achieve and maintain national certification in the Nurse Practitioner specialty.
Experience:
2 years within the last 5 years in chronic disease management/long‑term care. Hospice and/or palliative background preferred.
Required:
Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80–100 lbs.
