We are seeking a compassionate Licensed Behavioral Health Professional to join our integrated care team at the IHC Community Health Center of Bendix in South Bend. The role involves providing comprehensive behavioral health services within the Primary Care Behavioral Health model.

Job Responsibilities
Provide comprehensive assessment and diagnosis of behavioral health patients
Assist providers in recognizing and treating mental, psychosocial, and substance abuse disorders
Develop effective treatment plans and assist patients in achieving goals
Evaluate crises and apply appropriate interventions and referrals
Provide assessment, consultation, and brief intervention for psychological/psychiatric disorders
Maintain an active presence and communicate with PCPs during clinic hours
Available for same‑day and scheduled initial interventions with patients
Perform brief, limited follow‑up visits for selected patients
Actively participate in interdisciplinary meetings to support IHC’s integrated healthcare model
Assist in detecting “at‑risk” patients and developing preventive plansHelp primary‑care team develop care‑management processes such as guidelines, disease‑management techniques, case management, and patient education to improve self‑management of chronic disease, including substance use
Teach patients, families, and staff prevention and treatment enhancement techniques
Monitor the site’s behavioral health program, identify service‑related problems, and make recommendations for improvement
Desired Skillset
Excellent working knowledge of behavioral medicine and evidence‑based treatments for medical and mental health conditions
Comfortable with the pace of primary care and interdisciplinary teamwork
Ability to design and implement clinical pathways and protocols for chronic conditions
Proficiency in brief patient contacts and rapid, accurate clinical assessment
Strong communication skills
Cultural awareness and sensitivity
Good knowledge of psychopharmacology
Requirements
Master’s degree in social work from an accredited university/college
LCSW, LMFT, or LMHC licensure in Indiana
Basic Life Support (BLS) certification (AED included)
Active, clean Indiana driver’s license with state‑mandated auto insurance
At least 1 year of experience (3 years or more preferred)
