Case Manager V – Institutional (GID)
Texas Juvenile Justice Department – Bastrop, TX
Overview Perform highly advanced case management for youth in institutional settings, maintaining long‑term contact with families and service providers.
Responsibilities
Develop and maintain case files, conduct assessments, and create individualized plans.
Coordinate service delivery, evaluate outcomes, and ensure program compliance.
Supervise and mentor junior case managers as needed.
Collaborate with facility staff on schedules, safety protocols, and activities.
Attend pre‑service training academy when required.
Qualifications
Relevant bachelor’s degree or equivalent experience in social work, juvenile justice, or related field.
Minimum of 3 years of case management experience preferred.
Strong communication, critical thinking, and organizational skills.
Ability to work flexible hours, on‑call, and in high‑stress environments.
Pass a criminal background check and meet all minimum qualifications.
