Job Summary Pertinent information is gathered from incoming calls concerning medications, and, along with reviewing the electronic medical record, call‑center employees utilize scripting to determine a response to the caller’s needs. Responsibilities include addressing medication‑related questions and issues, handling prior authorizations, and approving refills. Employees perform a medication review at the time of the call, conduct a complete clinical review, and address any medication‑related questions.

Job Duties

Support the overall goals and objectives that result in improved access, quality of patient care, and higher levels of patient satisfaction.

Demonstrate proficiency in order processing.

Receive and interpret physicians’ medication orders and clarify or correct orders with the physician.

Review patient medication profiles to guard against drug sensitivities, contraindications, duplications, and major drug interactions.

Collaborate with health‑system physicians under a collaborative practice agreement to review patients’ electronic medical records and authorize prescription refills on pre‑specified maintenance medications.

Conduct a thorough review of each individual patient’s electronic medical record and develop an appropriate plan according to the pharmacist refill authorization protocol and the most recent clinical guidelines.

Use clinical judgment to determine if additional information is required prior to refill authorization and take necessary actions to resolve issues within the terms of the collaborative practice agreement.

Document encounters within the electronic medical record; e‑scribe approved prescription refills to the appropriate pharmacy and notify patients of completion.

Participate in the creation and update of scripting and documentation materials.

Education Bachelor’s Degree in Pharmacy (Required). Doctoral Degree (Preferred).

Experience Licensed Pharmacist in Pennsylvania required within 90 days of hire. Licensed Pharmacist – Default Issuing Body.
