The Assistant Vice President, Care Management is responsible for providing support, service, leadership, and oversight for critical aspects of patient case management performed by affiliated entities ensuring regulatory compliance, developing and implementing educational resources, and providing standardized leading practice to the field.

How you’ll contribute An AVP Care Management who excels in this role:

Support and assist Case Management/Care Coordination/Transition of Care and resource utilization efforts throughout the LifePoint System to improve care processes and patient outcomes.

Collaborate and coordinate with the National Medical Director to develop and implement the Hospital Support Center (HSC) care management strategy for the Company and respective divisions of facilities.

Evaluate pertinent Case Management metrics within division facilities, identify opportunities for improvement, and facilitate actions to address the priorities at the facility or division level.

Conduct onsite Case Management programs assessments with facilities and potential new affiliates; Work with hospital Case Management and leadership teams to track, trend, educate/train, monitor, and develop effective corrective action plans.

Maintain open communication with Health Support Center (HSC) division and department leadership team regarding performance of individual hospitals, gathering feedback for further improvement opportunities and strategies.

Collaborate with internal HSC departments to develop and present targeted education to optimize compliance with regulatory and payer requirements which promote high-quality, cost-effective care.

What we’re looking for Applicants should have a Bachelor’s Degree in Nursing, Master’s Degree preferred.

Additional requirements include:

Experience : Minimum of 7 years of experience with 2-3 years as Director, Case Management.

Certifications : Certified Case Management (CCM) or Accredited Case Manager (ACM)

Licenses : Current licensure as a Registered Nurse in state of residence.
