ASAP Job Summary: We are seeking a compassionate and skilled
Registered Nurse (RN)
to provide exceptional care to residents in a Skilled Nursing Facility. The RN will assess, plan, implement, and coordinate individualized care plans, ensuring the safety, comfort, and dignity of each resident. This role requires hands-on patient care, accurate documentation in EMR, and collaboration with interdisciplinary teams. Essential Duties & Responsibilities: Assess residents’ needs and develop, implement, and update individualized care plans. Administer medications and treatments per physician orders and facility protocols. Maintain residents’ privacy, dignity, and self-esteem while promoting independence. Ensure safe and orderly resident living areas; promptly respond to call lights. Observe and report residents’ conditions to appropriate staff and physicians. Educate residents, families, and significant others on care plans and treatments. Utilize proper transfer equipment (gait belts, mechanical lifts, slide boards, etc.) safely. Comply with all applicable laws, regulations, and facility policies, including corporate compliance programs. Participate in safety, fire, and disaster preparedness as required. Qualifications: Education & Licensure: Graduate of an accredited school of nursing. Active RN license in the state of practice. BLS Certification required. Experience: Minimum of 3 months experience in a Long-Term Care (LTC) setting. Skills & Abilities: Strong communication and comprehension skills. Knowledge of nursing principles, medication administration, and patient care techniques. Ability to safely lift and move up to 35 lbs; proficient in patient transfer techniques. Effective documentation and EMR usage. Physical/Mental Requirements: Ability to move, position, and communicate effectively in a clinical environment. Accommodations available for qualified individuals with disabilities. Work Environment: Moderate noise levels typical of SNF settings. May require occasional travel based on operational needs.
