Job Title: Intake Coordinator
Salary: $100,000 per year DOE
Location: 1 day at Fountain Valley, 1 day at Garden Grove, 3 days at Tustin.

Position Purpose Under the supervision of the Clinical Manager, the Intake Coordinator is an essential member of the clinical team as a facilitator of clinical mental health assessments. The Intake Coordinator’s primary function is to facilitate the admissions process which includes risk assessments and appropriateness for services (both therapy and psychiatry).
Core Duties and Responsibilities
Provides clinical assessment to determine appropriateness of services using screening questionnaires and risk assessments
Coordinates of intakes for therapists and psychiatry providers
Conducts record reviews (ROI, custody paperwork, etc.)
Consults with Clinical Director, Manager, or Supervisor as needed
Reviews internal referrals weekly, conducts outreach to referrals within 5 business days, documents attempts, and closes the loop by informing providers of outcome of referral
Addresses urgent referrals within 2 business days and closes the loop by informing providers of outcome of referral
Communicates with administrative team to ensure review of patient’s insurance and eligibility coverage and informs the patient of copay/good faith estimate
Provides individual and group therapy, as needed
Ensures all appointments are properly created, scheduled, and confirmed
Manages waitlist and waitlist outcomes
Attends meetings as applicable to the job title. (i.e. all staff meetings
Behaves professionally and works collaboratively with co-workers to increase productivity
Refers therapy and psychiatry patients to outside specialists or agencies as needed, using clinical judgment and higher level of care parameters
Responds to crisis situations and intervenes therapeutically as necessary; assesses for safety of patient and assesses for suicidality, homicidality, and self-harm ideation
Completes documentation within organization policy of 72 hours
Assists with quality measures
Provide clinical supervision to ASW and MSW interns
Performs other duties and projects as assigned by the Clinical Manager of Mental Health
Education, Qualifications, and Experience
LCSW required
Bilingual: Spanish required
Minimum 3 years in related experience
2 years post licensure required
Experience in clinical intake and assessment processes
Strong computer skills, acute attention to detail, confident and professional communication.
Knowledgeable in general EMR/EHR systems is preferred.
