This is a
high-visibility, executive-level Case Management leadership role
within one of the largest and most complex acute care hospitals in the Southwest. The Case Management Director will lead hospital-wide care coordination, utilization management, discharge planning, social work, and chaplaincy services for a
790-bed, Level II Trauma Center
with exceptionally high patient volumes. This position requires a proven, data-driven Case Management leader with strong physician relationships, post-acute partnerships, and the ability to drive performance across a large, multidisciplinary team. Position Overview
The Case Management Director has full operational accountability for all Case Management functions across the hospital, ensuring compliance with regulatory standards while optimizing patient throughput, length of stay, and resource utilization. This leader works closely with executive leadership, physicians, and external partners to ensure continuity of care and measurable clinical and financial outcomes. The role demands executive presence, strong change-management skills, and the ability to independently analyze data, develop action plans, and lead large teams through complex initiatives. Key Responsibilities
Provide strategic and operational leadership for hospital-wide Case Management, Social Work, and Chaplaincy services. Direct and evaluate department operations, including care coordination models, staffing, FTE management, onboarding, competencies, and technology utilization. Ensure consistent adherence to care coordination and discharge planning standards, including documentation quality and timeliness. Allocate resources to meet productivity, quality, and performance benchmarks. Oversee all core Case Management functions and professional practice standards. Partner with finance and system Case Management leadership to achieve established financial and operational goals. Analyze operational and performance data to identify trends, gaps, and improvement opportunities. Lead sustainable performance improvement initiatives aligned with organizational goals. Ensure compliance with all internal policies, procedures, and regulatory requirements. Build and maintain strong relationships with physicians, executive leadership, legal, ethics, compliance, ancillary services, and community partners. Provide leadership during high-volume periods and participate in weekend on-call rotation as operational needs dictate. Leadership Scope
