Compensation & Benefits Annual Salary:

$140,000 – $160,000 (commensurate with experience)

Bonus Structure:

Percentage of collections

Position Overview We are seeking a Nurse Practitioner or Physician Assistant to provide full‑spectrum outpatient primary care to patients across all ages. This role emphasizes continuity of care, patient education, and improving health outcomes within our community.

Key Responsibilities

Conduct comprehensive physical exams and review diagnostic results

Diagnose and treat acute illnesses (e.g., infections, minor injuries)

Manage chronic conditions including diabetes, hypertension, heart disease, and obesity

Develop and implement individualized treatment plans

Provide patient‑centered care addressing physical, emotional, and social needs

Collaborate with physicians, nurses, and specialists for coordinated care

Educate patients and families on conditions, treatment options, and wellness strategies

Coordinate referrals, follow‑ups, and hospital admissions when necessary

Maintain accurate, timely documentation in the EMR

Lead wellness screenings and preventive health initiatives

Maintain current clinical knowledge through continuing education

Required Qualifications

Active Nurse Practitioner or Physician Assistant license in Arizona (or eligibility in progress)

Graduate of an accredited U.S.-based NP or PA program (Puerto Rico programs not accepted)

Current or eligible DEA registration

BLS certification required; ACLS preferred

Minimum 1 year of primary care experience preferred
