Qualifications:

* Active Licensed Practical Nurse (LPN) license

* Compassionate and resident-focused

* Strong communication and teamwork skills

* Ability to maintain accurate documentation

* Reliable, professional, and eager to grow

What You'll Do

* Administer medications and treatments per physician orders

* Monitor and document resident conditions and vital signs

* Support clinical care plans and provide direct resident care

* Perform wound care, catheter care, and assist with ADLs

* Collaborate with RNs, physicians, CNAs, and the full care team

* Provide education to residents and families

* Ensure compliance with facility and regulatory standards

* Respond to emergencies and provide timely, skilled care

* Assist with training and mentoring CNAs
