* Conduct comprehensive assessments of patients' health status, creating individualized care plans based on their needs and goals.
* Administer medications and treatments as prescribed, ensuring patient safety and comfort.
* Educate patients and their families about health management, disease prevention, and self-care techniques.
* Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate care and address patient needs.
* Monitor and document patient progress, adapting care plans as necessary to optimize health outcomes.
Candidate requirements
* A Bachelor of Science in Nursing (BSN) is required; an Associate Degree in Nursing (ADN) will be considered with relevant experience.
* Current Registered Nurse (RN) license in the state of Texas.
* Minimum of 2 years of clinical nursing experience, with at least 1 year in home health or a similar setting.
* Strong clinical assessment skills and the ability to work independently.
* Excellent communication and interpersonal skills to engage effectively with patients and families.
