Registered Nurse - Case Management
TLC Nursing Associates, Inc.
TLC Nursing Associates, Inc. is seeking a proactive and detail-oriented Registered Nurse - Case Management to coordinate and manage comprehensive care plans for patients across various settings. The Case Management RN will work closely with interdisciplinary teams to ensure quality, cost-effective care and smooth transitions across the continuum of services.
Job Responsibilities
* Perform patient assessments and develop individualized care plans in collaboration with healthcare teams.
* Coordinate discharge planning, referrals, and post-acute services to support continuity of care.
* Evaluate patient needs, eligibility for services, and available resources.
* Monitor patient progress and adjust care plans as necessary for optimal outcomes.
* Communicate with patients, families, and providers to ensure understanding of care options and resources.
* Document all case management activities accurately and in compliance with regulatory guidelines.
* Promote patient advocacy, education, and empowerment throughout the care process.
Qualifications
* Active RN license in good standing.
* Associate or Bachelor's degree in Nursing required; BSN preferred.
* Minimum 2 years of recent RN experience , with at least 1 year in case management or discharge planning preferred.
* Knowledge of managed care, insurance processes, and community resources.
* Strong communication, problem-solving, and organizational skills.
* Certification in Case Management (CCM or ACM) preferred.
