- Facility Setting: Acute Hospital
- Unit/Department: Case Management
- Shift: Day 5x8-Hour (08:00 - 17:00)
Requirements
- License: Active CA license (Compact: No)
- Certs: BLs
- Experience: 3 years
- EMR: Epic
- Compliance: Two supervisor references required from within the last year.
- Locals: Locals accepted
- RTO Policy: RTO must be 7 days or less and not within the first 2 weeks of the assignment.
Responsibilities
- Coordinate patient care and facilitate communication among healthcare providers.
- Develop, implement, and manage individualized care plans.
- Monitor patient progress and adjust care plans as needed.
- Ensure compliance with CMS, HMO, and Medicare regulations.
- Facilitate discharge planning and post-discharge follow-up.
- Utilize InterQual Criteria and Milliman Guidelines for patient assessments.
