CERTIFICATION & LICENSURE:
* LVN/LPN of California
* Active CA license
* 1 year of case management experience in acute or outpatient setting
* Knowledge of HMO and Medicare for the inpatient, home health, and SNF settings
* Travel Experience Required
PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:
* Minimum of 2 years of acute care experience
