Qualifications
Must be licensed as a registered nurse or licensed practical nurse in the state of Arkansas.
Minimum of one year of hospital practice preferred.
Physical: this is a safety‑sensitive position. Normal hospital environment. Occasional exposure to fumes, odors, biological and electrical hazards. Normal/corrected eyesight with close eye work. Hearing of normal and soft tones. Uses computer, telephone, copier, fax, scanner and adding machine. Lifting up to 50 lbs, carrying up to 20 lbs, pushing/pulling up to 100 lbs.
Must be aware of the St. Bernards Substance Abuse Policy.
Job Summary The Admit Discharge Nurse is responsible for the admission interview, medication reconciliation, and discharge process. The position must possess excellent communication skills and function at a high level as a member of the collaborative team. This role is pivotal in coordinating the care of the patient pre‑ and post‑discharge to facilitate an efficient and effective acute care stay, and coordinate accurate information to the next provider(s) of care to minimize readmissions. This position requires independent judgment.
